Ministry In-Take Form

Oasis Prayer Ministry offers private prayer sessions with Sarah and/or Todd Cramer (and occasionally one of our trained volunteer prayer ministers). If you would like to schedule a prayer appointment, please complete the Personal Intake Form below. The Cramers have over 20 years experience in prayer, training, and spiritual formation. Each session costs $150 to support this ongoing ministry. This can be paid in advance through the payment key on this website (coming soon) or through arrangements with Sarah Cramer.

The forms are stored in a secure location and are kept CONFIDENTIAL. The forms are used to inform your prayer minister(s) of your current prayer needs before your arrival. We look forward to praying with you!
